8E | The Denial Before the Claim: Using Prior Authorization as an Outpatient Facility Revenue Signal
For hospital outpatient departments, prior authorization is more than an administrative checkpoint. It is one of the earliest places an organization can see that revenue may be at risk. An authorization denial or request for additional information can expose medical necessity concerns, missing clinical evidence, site-of-service conflicts, or payer requirements that may reappear when the facility claim is submitted.
This session examines how outpatient facilities can use prior authorization activity and denial data as leading indicators rather than waiting for a claim to deny. Beginning with current clinical denial trends, the discussion will show how authorization data can reveal risk by payer, CPT/HCPCS code, service line, and denial reason.
The session will also examine what CMS-0057-F changes for outpatient facilities in 2027 and what it does not, including why faster electronic decisions and greater access to payer requirements will not guarantee payment. Attendees will see how AI can help compare clinical documentation with payer policies, identify recurring patterns, and route exceptions for human review so that what is learned from one case can help prevent the next denial.
Learning objectives
Attendees will be able to:
-
Explain how prior authorization activity can provide an early warning of outpatient facility revenue risk.
-
Identify authorization patterns involving medical necessity, clinical documentation, payer requirements, and site of service that may lead to facility claim denials.
-
Distinguish the improvements introduced by CMS-0057-F from the payment and denial risks the mandate will not resolve.
-
Describe how denial analytics, AI, and human review can be used together to move prevention ahead of claim submission.
